Record Memo

This form can be used to record a memo against this user's record. Is is intended for use purely to record the reasons for changes the user's personal data. For example, that the surname has been changed from Smith to Brown because the user has married and asked for the change to be made. It is possible that the user will be given access to all memos made under their name.

Type a short memo into this box. This memo will be recorded as an event against the selected user. The event will also be recorded with the current date/time and your user id.
