When you select the button above a window will appear, containing the questionnaire. If the questionnaire is for review only you can read it and then close the window. If the questionnaire is ready to complete you will find buttons to select and boxes to type comments in. When you have finished you must press the button at the bottom of the page to record your responses. Until the owner of the questionnaire closes down access you can revisit it and adjust your answers.
Your answers are recorded on the Bodington system database against your login name and this allows you to revisit the questionnaire and review your answers. However, academic staff who create questionnaires do not see your login name or real name when they review your answers. (They may sort the data according to parent department or degree programme.) So the questionnaire is, in effect, anonymized.
